Each case is assigned to a Professional Care Manager, and each Care Manager guides the family through the following process:
Liken’s home care process begins with an Initial In Home Review conducted by a professional Care Manager and designed to meet the specific needs of both the family and the individual consumer. This is done with a personal visit to the home or health care facility by one of Liken’s Care Managers; once completed, the Care Manager will discuss the assessment with the family and the consumer and any other professional that needs to be included and a Plan of Care will be developed. The meeting will cover a range of issues including health, security, emotional, financial situations and legal requirements that need to be met.
Liken’s Care Manager will then create a tailored care plan based on the above meeting specific to each persons needs. The plan then will be reviewed with everyone involved; the family, the consumer, and anyone else necessary. We will then coordinate and manage the appropriate services.
Coordinated by the Care Manager, once the care plan is approved, we begin our search for the best person to implement that care. Our process takes numerous factors into consideration; the care services needed consumer interests and the personalities of the family, the consumer, and the caregiver.
Our caregivers keep extensive notes on the care they give to the consumer. Their activities are closely supervised by the assigned Care Manager both in the field with unannounced visits and in the office. All caregivers are evaluated annually along with ongoing training provided to make sure they are providing the maximum in professional caregiving to the consumers and their families.
